We wish everyone good luck on your hockey season. 

The following terms will apply for ALL classes.

General Conditions

	All Children must wear full hockey equipment to participate.
	Full Hockey equipment is mandatory. Children will not be permitted on the ice without this equipment..
	Proof of registration with LMHA or Rosedale or another association, will be requested.
	After a short evaluation period, we will move children up (or down) to the class that will benefit them most.
	Registrations will be accepted on a first come, first served basis only. (Lawfield & Rosedale first)
	Class sizes will be limited. You will receive written confirmation of registration.
	If classes are full, you will be notified and your cheque destroyed, or returned to you at your request. A small waiting list will be  kept in case of cancellation.

Parents / Guardians - NOTE: - The following statements will appear on your confirmation form. 

I give my full approval to my child's' participation in all activities associated with this program. I knowingly recognize, accept and assume all risks and hazards incidental to such participation including medical and dental expenses. 

I certify that my child is in good physical  and mental health and is able to participate in any and all activities associated with this program.
